CORI Acknowledgement Form
This form is provided by the Commonwealth of Massachusetts Executive Office of Public Safety and Security is used for organizations requesting Criminal Offender Record Information (CORI). The subject of the CORI (the applicant) should be provided this form to acknowledge their understanding of the use of the CORI by the organization they are applying work or volunteer for. Organizations can use this form alongside the CORI Request Form to show the applicant’s understanding of the CORI request.
