Level 1 Evaluation Survey: Your Opinions Matter
This document provides an example of an evaluation survey based on Level One of Kirkpatrick’s Four Levels of Training Evaluation. The document allows trainees to reflect on their training experience and give feedback about the training. It can be used as is or as a template for developing your own training evaluation. Information from training evaluations like this allow trainers and leadership to ensure their training is effective or to make changes where needed.
