SORI Request Form
This form is provided by the Commonwealth of Massachusetts Sex Offender Registry Board and is used by organizations to request SORI or Sex Offender Registry Information during the applicant screening process. The form requires information about the applicant and the requestor, including their name, address, and some identifying information. It should be used to request information about applicants the organization is considering hiring, to ensure they do not have criminal backgrounds.
