CORI Request Form
This form is provided by the Commonwealth of Massachusetts Executive Office of Public Safety and Security is used for requesting Criminal Offender Record Information or CORI. It requires the applicants name, including former last names, birthdate, and other personal information. The form should be submitted during the applicant screening process. It should be used to request information about applicants the organization is considering hiring to ensure they do not have criminal backgrounds.
