51A Report Form
This official form from the Department of Children and Families is used to report alleged child abuse or neglect. This form includes space for a reporter to record all the information needed to make a report. It should be used when a mandated reporter or other person is making a report to the Department of Children and Families. These forms should be made readily available by leadership at Youth-Serving Organizations (YSOs) for their staff and volunteers to access in the case of suspected abuse or neglect.
