Sample Child Safety Incident After-Action Report
This is an example form to be used when an incident of child safety occurs at a Youth-Serving Organization (YSO). The form allows staff, volunteers, or leadership to fill out an official statement of an incident regarding child safety. This form includes details about the incident, including when it happened, who was involved, a description of the incident, and what happened regarding reporting the incident. It should be used for internal records and reflecting on incidents of child safety. Any person at a YSO should be able to use this form to record an incident.
